CARA MENGURUS AKTE KEMATIAN DI DINAS KEPENDUDUKAN DAN PENCATATAN SIPIL KOTA YOGYAKARTA, KABUPATEN SLEMAN, KABUPATEN BANTUL.

Sebelum ke Dinas Kependudukan dan Pencatatan Sipil , tentunya harus mengurus Surat Pengantar terlebih dahulu mulai dari R T- RW - Kelurahan, baru setelah itu ke Dindukcapil. Kepengurusan Akte Berdasarkan Asas Peristiwa

PERSYARATAN UNTUK MEMPEROLEH DOKUMEN AKTA KEMATIAN
  1. Asli Surat Kematian dari Dokter/ Rumah sakit/Petugas Kesehatan.
  2. Asli Surat Keterangan Kematian dari Kelurahan.
  3. Bagi penduduk yang meninggal di rumah, dilampiri asli Surat Kematian dari Puskesmas.
  4. Fotocopy KK dan KTP yang bersangkutan/yang meninggal 1 lembar
  5. Fotocopy KK dan KTP pelapor/ pemohon selaku ahli waris 1 lembar
  6. Membawa 2 (dua) saksi minimal umur 21 tahun dan dilampiri fotocopy KTP saksi.
  7. Fotocopy surat nikahyang meninggal.
  8. pengantar dari rt , rw, dukuh.
Keterangan :
  1. Syarat- syarat tersebut di atas berlaku bagi permohonan akte kematian belum terlambat maupun sudah terlambat.
  2. Apabila terlambat lebih dari 30 hari sejak meninggal, dikenai denda administratif .
  3. Apabila yang meninggal dunia adalah sebagai kepala keluarga, untuk Kota Yogyakarta maka diwajibkan untuk pisah KK terlebih dahulu di Kecamatan, dan fotocopy KK yang dilampirkan adalah KK setelah dipisah, untuk Kabupaten Sleman dan Bantul tidak.
MANFAAT/ KEGUNAAN AKTA KEMATIAN
  1. Sebagai alat bukti yang sah bahwa seseorang telah meninggal dunia.
  2. Dari segi praktis Akta Kematian dapat digunakan untuk :
  • untuk persyaratan pengurusan pembagian waris baik bagi istri atau suami maupun anak.
  • Bagi janda atau duda (terutama bagi peg. Negeri) diperlukan sebagai syarat nikah lagi.
  • Diperlukan untuk mengurus pensiun bagi ahli warisnya untuk mengurus uang duka (santunan kematian), Tunjangan kecelakaan, Taspen, Klaim Asuransi dan lain sebagainya.
